The rehabilitation of the atrophic edentulous maxillae is still a great challenge in implantology; invasisve grafting procedures are mandatory to perform this kind of treatment, if the conventional protocol is employed. The objective of this lecture is to describe an alternative protocol in which the rehabilitation of the edentulous maxillae is performed with no bone grafting procedures, with the utilization of conventional implants strategically tilted, and/or with the utilization of zygomatic fixtures. This approach represents a predictable option for full-arch, implant-supported rehabilitations, with a survival rate of over 97% for conventional and zygomatic fixtures.

Comprehend the problems associated with removable complete dentures in atrophic maxillae, and the physiological mechanisms of bone resorption that initiate after tooth extractions.

Understand the concept of tilted implants in the rehabilitation of the edentulous maxillae, and the biomechanical principles that allow the utilization of only four implants to support a full-arch fixed prosthesis.

Comprehend the evolution of surgical techniques for installation of zygomatic fixtures, and the benefits obtained with these advances.

Recognize the three techniques employed to rehabilitate edentulous maxillae without grafting procedures: All-on-4 Standard, All-on-4 Hybrid and All-on-4 Zygoma; and the indications for each of these techniques.
